NHS to develop mobile app for Coordinate My Care

NHS is developing an app as part of its Coordinate My Care service that will allow patients receiving end-of-life care to access their Coordinate My Care records, ensuring that they are receiving appropriate treatment.

NHS is developing an app as part of its Coordinate My Care service – which went live in London this spring – that will allow patients receiving end-of-life care to access their Coordinate My Care records, ensuring that their preference of dying at home or in hospital is fulfilled and that they are receiving appropriate treatment. The app will also support mobile working by professionals.

It was discovered that under 20% of those with chronic illness die at home, although that is preferred by most, and a computerised register of preferences accessible by all professionals likely to come in to contact with a patient was recognised as the best solution to deal with this.

To date, 5,300 patients have created a Coordinate my Care care plan and the scheme has received £1.5m in funding for the current year.

The plan is to coordinating care in all long term conditions – a major priority under plans announced by the government this week.
